Clinical Pharmacist

About the role

The Clinical Pharmacist will deliver safe, effective and rational medication‑use services within a hospital setting in Sri Lanka. The role involves reviewing prescriptions, providing medication counselling, supporting JCI accreditation readiness and contributing to quality improvement initiatives.

Key responsibilities

• Provide readiness for JCI audits and support accreditation‑related activities.
• Review prescriptions for appropriate drug selection, dosage, frequency, route and potential interactions.
• Conduct medication reconciliation and provide counselling to patients and caregivers.
• Identify and report medication‑related problems, adverse drug reactions and medication errors.
• Support antimicrobial stewardship and monitor high‑alert, high‑risk and restricted medicines.
• Participate in medication‑use audits, quality improvement projects and hospital safety initiatives.
• Collaborate with consultants, medical officers, nurses and other healthcare professionals.
• Maintain accurate pharmacy and clinical documentation and assist pharmacy operations as needed.

Required profile

• Bachelor of Pharmacy (B.Pharm) or equivalent recognized qualification.
• Registration with the relevant professional/regulatory authority in Sri Lanka.
• 1–2 years of relevant professional experience, preferably in a hospital or clinical pharmacy setting.
• Good clinical and pharmaceutical knowledge with strong attention to detail.
• Excellent communication and interpersonal skills, patient‑centred approach.
• Ability to work effectively within a multidisciplinary team and independently while maintaining accountability.
• Willingness to learn, develop professionally and contribute to continuous improvement of clinical pharmacy services.
